Dental operating microscope (D.O.M.), D.O.M. versus completely calcified systems, Root Canal Treatment Post-Therapy 483136-1

An intricate root canal procedure, because this pre operative condition involves dealing with complete canal stenosis caused by dystrophic calcifications.
Case Study Number 483136

Once located, with the help of a dental operative microscope (Opmi Proergo from Carl Zeiss), mesial canals have been shaped and cleaned with the Pro Taper system (Maillefer) and lots of RC PrepTM. Then, they were filled with gutta percha (lateral and vertical condensation) and Pulp Canal Sealer EWT TM

I am truly amazed with this root
canal treatment outcome (and by the whole website), surgical
microscope is definitly the way to go. Still I can’t help wondering
how the sealer overfill will affect the prognosis of treatment?

      Hi Dr Nguyen,
I am glad that you enjoyed visiting this blog. I am glad too that you brought up this overfill issue. To answer your question I will content myself citing Dr L. Stephen Buchanan, a senior lecturer in endodontics:” Endodontics is plagued by the difficulty we have in deconstructing our failures because everything is so small and hidden from direct view. Because of this, very few dentists understand that overfills are only associated with failure of treatment when apical damage as been caused during shaping procedures and the main etiological factor (infection) was not properly addressed. While I agree that apical extrusion of filling material should not be our clinical objective, when tapered shapping is well done and the canal has been sealed to its full apical and lateral extents, surplus filling material (while perhaps regretted for aesthetic reasons) will not lower the prognosis of treatment.” Reference: Buchanan LS Tapering shaping objectives can make your life easier Dent Today. 2010; Volume 29 No 1, 112-19

I sincerely hope this could help
