Conventional endodontic treatment is based on emptying, shaping, disinfecting and filling of the root canal system. Root canal shape can be challenging to the dentist, especially in teeth with severe root curvatures (small root curvature radius or apical hooks). Transportation of the apical foramen, creation of ledges, elbows, zips, perforations, and instrument fracture can occur along with loss of working length.
Thus careful planning of endodontic treatment are mandatory to reduce failure rates.
Furthermore, this patient is young, thus root canal space are large. Shaping with a MAF size 25 and smaller, in this case, would not be large enough to remove all the pulp tissue in the apical area. Leaving some infected tissue behind may lead to a root canal failure. Thus a Profile ISO 0.04 Taper size 35 was the elected one (size to which no more blood and pulp tissue could be seen on the instrument flutes as well as no more blood on absorbant paper points).
